***Please note: a standard release form for medical information may be obtained from most medical offices. If you do not have a current physician and are unable to obtain a release form, a written and signed letter of request for copies of medical charts will suffice.

| Former patients with family physicians will not require chart copies because the referring physician always receives copies of all letters, operative notes and laboratory reports from each patient encounter. For patients who require their charts to be sent to another physician, please mail signed letters of request or standard release forms from the new physician with patient contact phone number, to: Dr. Ian Dale c/o The Hamilton Academy of Medicine, 1 Young Street, Suite 206, Hamilton Ontario, L8N 1T8. Include a cheque with your release form made payable to: Dr. N. Ian Dale for $80. Once payment has been received, arrangements will be made for delivery of copied charts to the new physician. | Patient Charts will be stored safely and securely in a storage facility for ten years, as required by law. If, for any reason, you or your physician require a copy of your record, photocopies will be available upon written request sent to: P.O. Box 182, Carlisle, ON, L0R 1H0. Retrieval, photocopying and postal charges will be applied. For ongoing medical care, please see your family physician. |

| For copies of patient charts, patients with a family doctor do not require chart copies, as the referring physician always receives copies of all letters and endoscopy reports. Also, copies of all notes and tests done are added to the patients chart at St. Joseph’s Healthcare and are available through the Health Records Department. For patients who require their chart, please mail a written signed letter of request or standard release form to: Dr. R. | For any patient medical records left at 60 King Street West in Stoney Creek, ON, they have been transferred to "Records Storage & Retrieval Services (RSRS)". To obtain a copy of these records, please call 1-888-563-3732, ext. 1. Note that on January 31, 2011, in accordance with the rules and regulations of The College of Physicians and Surgeons of Ontario, these patient records will be destroyed permanently. |
